This listing is for Private Practitioners who are SPA members to list their clinics on SPA website. Please fill the form below to subscribe to the directory listing.
Eligibility:
- Where private practice clinic has more than one AHPC registered physiotherapists, to qualify for listing, at least 50% of the physiotherapists must have valid SPA membership.
- Name of Physiotherapist(s) to be listed must have valid membership with SPA.
- An annual fee of $50 is chargeable per clinic address listing and subject to annual review by SPA.
Please note:
- The SPA council will review your application and reply within 4 weeks with the result of your application.
- The listing will expire a calendar year from the joining date. You will receive a renewal reminder 30 days before the expiry.
- By completing and submitting this form, you are declaring that the above criteria are met and the information submitted are true on day of submission.
- SPA reserves the right not to list the clinic, at its own discretion, if the criteria are not met.

Therefore, it needs to be a VALID and REGULARLY ACCESSED email as all the notifications including renewal reminders and invoice will be sent to. The Association will not be liable for any missed communication due to incorrect or not-in-use email address entered below.
